Frontera Diabetes Services – El Paso, Texas
El Paso is our home — and diabetes has touched nearly every family on this border. Frontera Diabetes Services was created to change that, right here, for our community, by people who understand what it means to live on this frontera.
We offer free, bilingual diabetes education and prediabetes screenings in the churches, community centers, and neighborhood spaces where El Paso families already feel at home. Our Diabetes Self-Management Education (DSME) curriculum follows the 10 nationally recognized content areas established by the Association of Diabetes Care & Education Specialists (ADCES) and is delivered by trained, DSHS-certified Community Health Workers — Promotores(as) de Salud — who are members of our own community.
Valance Wellness Institute holds a Medical Supplies and Devices License with the Texas Department of State Health Services, allowing us to provide glucometers, testing strips, and diabetes supplies directly to participants who need them.
We are also in the process of obtaining CDC recognition for our Diabetes Prevention Program — the proven national standard for cutting the risk of developing type 2 diabetes in half.
Community-Centered Approach
We know that managing diabetes is about more than a diagnosis. It is about whether you can afford food, get to a clinic, or find someone who will explain your health in your language without rushing you out the door. That is why every participant receives a social needs assessment at enrollment, and we connect families directly to food, transportation, housing, and insurance resources — because nothing should stand between you and the care you deserve.

“Support Diabetes Prevention”
Many people in our community live at risk of developing diabetes without health insurance or access to preventive services. Frontera Diabetes Services provides preventive health education (DSME) and assessments to help people detect, prevent, and manage diabetes.
